• Provide professional, respectful and ethical individual and group counseling to adults using a person-centered approach.
• Function effectively and collaboratively with all other members of the clinical team, administrative and support staff, and attend daily clinical staffing.
• Professionally assess patient needs and make appropriate referrals for dual diagnosis, mental health, physician, outreach programs, recovery houses and continuing care plans.
• Document consumer progress in a timely and accurate manner. This includes treatment plans, progress notes, case presentation and discharge summary, all based on SCVRD criteria.

• Attend agency trainings and perform lectures/outreach projects/programs as requested.
• Maintain collaborative working relationships with agencies invested in the consumers' success to enhance their experiences in receiving all necessary referrals and services.

• Conduct various outpatient services to include individual therapy, group therapy, and crisis interventions with the goal of patient recovery.
• Complete patient intakes, follow up on referrals, schedule appointments, and reviewed orientation documents with patient to include information about patient privacy.
• Develop individualized service plans and complete reviews/assessments as needed throughout treatment.
• Efficiently managed a high-volume caseload and completed/maintained patient documentation.
• Collaborated with psychiatrists, nurses, and doctors to create treatment plans to include psychotropic medications.
• Completed prior authorizations requests for patient insurance providers for approval of services.

In conjunction with first responders, we provided in-person crisis intervention services to individuals in need within the community.

Successfully managed assigned caseload and provided effective vocational rehabilitation services to consumers. This internship was supervised.
