Tailored Day Services Instructor
Purpose: This role closely works with Individuals with intellectual and developmental disabilities. Under the direction of the Program Coordinator a Community Instructor supports clients in meeting the goals outlined in their initial intake and ISP's all while maximizing the amount of program hours authorized by their Service coordinator.
Responsibilities Include:
- Reviews client's initial skill assessment and/or profile and individual service plan with the assistance of the coordinator or in-house counselor
- Provide clients with the support and training necessary to meet their goals, desires, and aspirations
- Assisting with locating needed benefits and services Communicating important information to the support team of the individual (Parents, Case manager, program manager, SC's)
- Effectively plan weekly community activities based on the client's individual plans and/or choices
- Ensures that clients meet the production and quality control needs at various employment and/or volunteer sites.
- Adheres to, enforces with clients, and ensures knowledge of safety standards at employer/job sites and in the community
- Provides data collection related to individual clients that reflect progress, problems, solutions, and other pertinent data
- Maintain the health and safety of individuals while in the community and traveling to community sites
