• Conduct comprehensive assessments of family strengths, needs, and risks at the time of a child’s
removal or placement into alternative care.
• Gather medical, developmental, educational, and psychological information from all relevant
sources to inform case planning.
• Document all assessments in the FACES system within required timeframes.
• Arrange for safe, appropriate alternative placements (foster, kinship, or residential) that best meet
the child’s individual needs.
• Conduct regular home visits to monitor safety, stability, and the child’s adjustment in placement.
• Ensure children receive timely and ongoing medical, dental, and behavioral health services.
• Collaborate with foster parents and resource providers to address care concerns and support
placement stability.
• Ensure each child receives a health screening within 72 hours of placement and ongoing
preventive care per state policy.
• Monitor and coordinate behavioral health services, including therapy, psychiatric care, and
medication management.
• Maintain up-to-date medical and behavioral health records in compliance with Missouri Children’s
Division policy.
• Facilitate Family Support Team (FST) meetings to develop, review, and update service plans and
permanency goals.
• Engage parents, kinship caregivers, and community supports in case planning and
decision-making.
• Coordinate and supervise family and sibling visitation as ordered by the court or case plan.
• Provide parenting education, coaching, and referrals to help families address barriers to
reunification.
• Develop and maintain case plans that support timely permanency outcomes—reunification,
adoption, guardianship, or another planned permanent living arrangement.
• Conduct concurrent planning to ensure multiple permanency options are actively pursued.
• Monitor and document family progress toward service plan goals.
• Prepare detailed court reports, case summaries, and recommendations for judicial review hearings.
• Testify in court proceedings regarding child safety, well-being, and permanency progress.
• Ensure compliance with court orders and follow up on mandated services and timelines.
• Respond promptly to placement disruptions, safety concerns, or emergencies involving children or
caregivers.
• Coordinate emergency placements and services as needed.
• Maintain availability for after-hours or on-call support according to agency policy.
• Maintain complete, accurate, and timely documentation in all assigned cases in accordance with
Children’s Division policies.
• Record case contacts, FST minutes, service referrals, and progress notes in FACES and other
required systems.
• Monitor and update service and treatment plans to reflect current family and child needs.
• Coordinate services and communication with community providers, schools, juvenile courts, law
enforcement, and medical professionals.
• Advocate for children and families within multi-disciplinary settings.
• Serve as liaison between caregivers, biological families, and service providers to promote
consistent, coordinated care.
• Adhere to Missouri Children’s Division practice standards, including trauma-informed and
family-centered approaches.
• Ensure all actions and case decisions comply with state statutes, agency regulations, and federal
mandates (e.g., ASFA, ICWA).
• Participate in supervision, case staffing, and required professional development or training.
