Develops, conducts and completes patient assessments timely, thoroughly and accurately to recover reimbursement and avoid overpayment in accordance with the requirements of state, federal and company guidelines.  Partners with the therapy department and acts as a patient liaison/advocate from  the organizational level to the corporate level related to the appropriateness of the prevention of denials for insurance companies.  Participates in discharge planning, care plan and quality assurance meetings, as well as the appeal process when a denial letter has been issued.  Collaborates with all members of the  interdisciplinary team.  Knowledgeable in PASRRs.  Develop and implement nursing care plans for patient and coordinates with multidisciplinary team members to facilitate integrated and comprehensive care.  Educates, trains and directs staff on the requirements of accurate supporting documentation for the MDS.

Assess home care patients, identifying physical, environmental ad psychosocial needs.  Assessment of all medications to identify possible adverse reactions, drug allergies and contraindications.  Establish realistic measurable and observable goals, consistent with patients diagnosis.  Participate in interdisciplinary patient care conferences and communicate with the physician of any significant findings, problems or changes.  Skin and wound care. Glucose testing.  Accurate OASIS collection.  Patient and family education.

Communicate with patients, families and other members of the healthcare team regarding the patients care.  Use clinical reasoning to provide the best possible nursing care.  Accurately and promptly implement physician orders. Skin and wound care.  IV maintenance and administration.  Medication administration.  Patient and family education/teaching.  Give quality, patient-focused care in a compassionate and professional manner. Assessment and documentation.  Call in prescription refills to the pharmacy and obtain prior authorizations as needed.
