• School Nurse overseeing Esparto Elementary School, Esparto Middle School, and Esparto High School. Approx 1000 students.

*coordinates care and collaborates with multiple disciplinary team members across continuum of care.

*evaluates and identifies knowledge gaps of disease process and treatments

*involves telephonic case management as well as direct patient contact either through office visits or home visits

• Senior nurse for the Child Protective Service (CPS) Medical Neglect unit. Aids social workers in the Emergency Response, Informal Supervision and Hearts for Kids programs.
• Perform developmental assessment and appropriate referrals for children < 5 years of age in foster care
• Conducts home visits and assists the social workers in their investigations of child abuse, and on CPS referrals which present with nursing or medical concerns/conditions

• Review medical records for all new intakes in EI and help determine eligibility for services
• Provide consultation to service coordinators regarding special health care needs of infants with developmental disabilities

• Home visiting public health nurse(case manager) with emphasis in high risk prenatal, high risk infant and children, and Child Protective Services

• Home visiting nurse (case manager) seeing high risk pregnant women, infants, and children
