Identify and Close gaps related to discharge needs: Home Health, DME, Transportation, Meals, Disease mgt/Medication education, COPD/CHF zone action plan
Schedule follow-up visit with PCP within 3-5 business days of hospital discharge
Weekly follow-up calls or in-office visits with members during the first 4-6 weeks of the hospital discharge
Daily Clinical Huddles with PCP Review members discharged from hospital: reason for admission, follow-up needs, follow-up appt date
