
Bedside shift report, obtain vital signs, obtain bedside blood glucose levels, head to toe assessments, focused assessments, call
the appropriate healthcare provider as needed, obtain verbal orders from the healthcare providers, give medications through PO, IV, IM, SUBQ, tube feeds, and ID, chart appropriately for each patient, start IV, discontinue IV, patient and family educations, lab draws or venipuncture, blood and blood product transfusion, involve other interdisciplinary team members as needed, change patients who are soiled, wound care or dressing change.
Educate parents and patients regarding the patients’ diagnoses, discharge educations to the patients and family members, medication educations to the family members and patients, give medications through IV, intramuscular, intradermal, subcutaneous, oral, NG tube, G tube, and tube feeds, call the appropriate interdisciplinary team members as needed depending on how the patient is doing, shift change reports on the patients, receive and give reports to different department nurses for appropriate patients for the appropriate tests, procedures, and surgeries, give tube feedings through the tube feeds, discontinue IV from the IV site upon giving discharge education and paperwork to the families and patients, full head-to-toe assessments, focus assessments, give CHG wipe down on patients with central venous catheters, give soap and water baths on all patients that are under my care, weigh the patients, measure the abdominal and OFC girths as ordered by the healthcare providers, etc.
Take vital signs of the patient, assist with peri care, CHG wipe downs on patients with PICC or Central lines, obtain height and weight of the patients, obtain the urine and stool samples for the lab orders when requested by the RN, sitting for the patients who have 1 to 1 sitter orders, order the patient’s food orders to the proper channels, fill in as the PCT/CNA or sitter on the units that need help, and be a HUC as needed.
Fill in the CNA or sitter role on the units that need CNA or sitter, obtain the patient’s vital signs, assist with peri care, assist with
ADLs in those who need help, oral care, obtain urine and stool sample as told by the nurses, assist the nurses as long as it is within my scope of practice, answer call lights, relieve the sitters for breaks, take the patients to the medical imaging if needed, put the patients on the telemetry box, take the post op vitals, and etc.
Assisting patients with ADLs, provide direct care to patients, take and record patient’s vital signs and blood sugars, monitor and
record the intake and outputs. Obtain specimens, such as the non-invasive bodily fluids and blood specimens, perform EKGs. Monitor and perform actions to decrease patients’ length of stay, such as assisting patients getting dressed, putting all the patient’s belongings into one bag, and communication discharge information to the appropriate staffs. Assist the physicians and nurses with the diagnostic tests, clinical procedures, and other tasks. Communicating with the patients with their needs, report any changes in the patient’s conditions to the nurses and/or physicians.
Provide basic hygiene needs, assist with nutritional intake, collect patient specimens, collect vital signs, input/output, report any
changes in patients’ health status to nurses, provide EKGs, perform proper pre-op shaves and showers for patients,
