Currently working on a Medical Surgical Floor providing care for up to 6 patients with a variety of chronic medical conditions including Diabetes, Chronic Lung Disease, CHF, Pneumonia and COVID infections. Work closely with a interdisciplinary team with includes, Resident Physicians, Students, Attending Physicians, Physical Therapists, Dieticians and other Nurses.

Provided follow up discharge teaching to patient's recently discharged from the hospital who were of high risk for readmission. Assisted with setting up resources for these patient's in the community and making sure they had follow up appointments and transportation.

Worked in the hospital as a discharge planner setting up a safe discharge plan for patient's returning home from the hospital. Including setting up Homecare Services, ADL equipment and supplies and sometimes making referrals for SubAcute Rehab. Also did utilization review to provide documentation for continued stay qualification.

Worked on a Medical-Surgical unit providing care for up to 6 patient's. Often worked as a Charge Nurse on evenings and weekends. Precepted new nurses and worked closely with Residents, Students and Attending Physicians.
