RENAL WAITLIST TRANSPLANT COORDINATOR

  • Perform chart review, assessing the need for candidate re-evaluation, and initiating when appropriate
  • Identify potential problems, abnormal diagnostic findings, and/or significant changes post clinic visits and coordinate required care and referrals
  • Collects and evaluates medical, psychosocial, and financial data for review and formulates Medical Review Board packet for patient needing representation to the Review Committee for evaluation of risk vs benefit prior to proceeding with transplantation
  • Coordinate with interprofessional team the transplant evaluation update process for candidates in the waiting list including, but not limited to Medical/Surgical evaluation, Psychosocial evaluation, educational assessment, laboratory evaluation including serologies, tissue typing, and antibodies per protocol, radiological evaluation, and nutritional evaluation
  • Establish educational goals with patient and family/caregivers ensuring they understands their responsibility in proactive management in their health care while in the waitlist and provides education and appropriate resources regarding living donor options, Organ Procurement, Transplantation Network/United Network of Organ Sharing (OPTN/UNOS) and Kidney allocation system
  • Present waitlist class education to patient/family/caregiver post listing regarding 'Waiting' Process, Risk/benefits, benefits of living donor vs deceased donor, the utilization of expanded donors, high-risk donors, if applicable
  • Participate in outreach promotion of living donation education
  • Educate patients on the importance of PRA kits for antibody screen and that these are sent / obtained regularly per transplant center protocol and importance of maintaining routine health maintenance and surveillance for malignancy during the waiting period
  • Formulate recipient patient summary for Immunology Review Board discussion with living donor matching
  • Coordinate desensitization treatment per protocol for highly sensitized recipients
