As Clinical Coordinator
Responsible for performing coordinating and directing nursing care and services in a chronic care facility for ESRD patients in accordance with the company health care policies, procedures and guidelines, OSHA, CMS, Federal, state and local regulations.
Assists the Facility Administrator with the evaluation and counseling of clinical teammates as well as annual evaluation of teammates performance.
Assesses patient and family learning readiness and provides appropriate information on renal disease, prescribed dialysis treatment , and medical regimen and its impact on the patients states of health and wellbeing.
Provides dialysis therapy, including but not limited to formulating dialysis plan per physicians order, weight and rate of ultrafiltration, verifying safety measures are in place and complied with, administering medication or other appropriate treatments, discontinuing dialysis according to established procedure, documenting all activity appropriately.

Infection Control Manager:
Serves as resource for infection control information and care coordination
Focuses on infection control efforts and develops ways to improve care, to decrease incidence of infection, and to reduce incidence of morbidity and mortality
Maintains infection control data to provide current accurate tracking and trending.
Provides leadership and communication to verify infection control compliance.
Coordinates infection-related Root-Cause Analysis activities and collaborate on plans of improvement.
Conducts training and education by delivering clinical and technical knowledge for 8-12 weeks. Combination of classroom and hands on dialysis training specifically tailored to newly hired teammates with minimal or without hemodialysis training. This is to equip nurses and dialysis technicians necessary skills to provide excellent patient care.
Anemia Manager:
Implements anemia management protocol determined by medical director and nephrologist; review parameter and results to evaluate effectiveness of facility's anemia management program.
Reviews new patients admission labs and initiate anemia management and protocols as appropriate and as directed by physicians.
Maintains proactive communication with facility teammates regarding patient specific conditions, events, etc. related to the anemia management program.
Communicates with hospital acute programs to ensure continuation of medication and iron therapies during hospitalization.
Adequacy Manager:
Serves as the facility resource for adequacy management: to increase the percentage of hemodialysis patients achieving a minimum KT/V > or to 1.2 and a minimum URR > 65%.
Reviews and reinforces correct lab collection techniques prior to each monthly draws.
Reviews lab results with the Interdisciplinary team and help evaluate outliers and plans intervention prior to the next dialysis treatment.
Establishes ongoing frequent, formal/informal communication between all multi- disciplinary care team members regarding patient care or outcomes.
Vascular Access Manager:
Assures education of teammates, provider partners and hemodialysis patient in the care and management of vascular accesses with particular attention to areas with improvement opportunity.
Assures Monitoring and surveillance of the Vascular Accesses and the hemodialysis care delivery, with particular focus on physical assessment,access flows, pressures, etc. which may indicate early symptoms of access dysfunction.
Proactively collects and reports accurate and complete data assessment of the facility's access trends for baseline and ongoing status of vascular access care and management.
Provides leadership and communication to assure vascular access planning for current and future accesses.
Also served as a Preceptor and Star Program educator.
Delivers safe and effective patient care in compliance with established company standards; dialysis contracted policies and procedures, in- center renal services policies and procedures as well as regulations set forth by state and federal regulations.
