Complete biopsychosocial assessments to formulate and implement a treatment plan in collaboration with veteran, family, and interdisciplinary team.
Coordinate with veterans' family and Burial Detail Clerk to provide information about VA burial benefits and end of life planning.
Attend Interdisciplinary Care Plan Meetings and Unit Rounds in conjunction with Palliative Care Team.
Coordinate family meetings with veteran, family, and interdisciplinary team to address goals of care conversations and discharge planning.
Coordinate Vigil Volunteers to be with veterans at end of life.
Assist with gathering needed documents and completing Medicaid applications for community placements.
Complete bereavement calls to offer support and information encompassing end of life resources and benefits.
Value ongoing dialogue and the commitment to supporting veterans, families, and interdisciplinary team.
Seek and obtain supportive resources to support stability, homelessness, legal, employment, LTC needs, basic needs, etc.
Provide education and resources on advanced care planning/end of life planning.
Coordinate safe transitioning and discharge planning from hospital to home, community nursing home, assisted living facility, etc.
Advocate on behalf of veterans to promote independence, safety, self-determination, etc.
