Telephonic Nurse Case Manager

  • I performed Utilization Review as an embedded RN in the State Compensation Insurance Fund building with the claims examiners. We performed utilization review of workers compensation medical treatment requests based on the applicable CA guidelines. I provided assistance to the claims examiners with questions regarding medical treatment, appropriateness of care and indications for medications/treatment requests. I provided utilization review determinations based on the treatment guidelines or referred the request to a reviewing physician if the request was not supported by treatment guidelines.
  • When the State Fund contract with EK Health ended, I transitioned into the role of a Telephonic Nurse Case Manager, which I have been doing for the past 15 years. My roles have varied from being a dedicated TCM for specific carriers/employers to working in some of the other programs that EK Health provides, such as RN Pharmacy Review and the Specialty/Next Step Programs.
  • Coordinate with multidisciplinary teams to facilitate progress toward a return to prior level of function/work and/or to highest level of functional ability while also working to provide cost savings/reductions to the stakeholders.
  • Collaborate with insurance providers, vendors and the injured worker to facilitate authorized medical treatment services in a timely manner.
  • Help injured workers navigate complex workers compensation insurance processes while assisting with access to necessary medical treatment.
  • Provide timely updates/Case Management reports to stakeholders regarding progress with current medical treatment and provide recommendations when appropriate.
  • Enhance patient care by providing telephonic support and guidance to individuals with chronic pain/injuries.
  • Assess current treatment outcomes/progress to identify barriers, clarify or determine realistic goals and objectives and seek potential alternatives.
  • Participate in ongoing professional development opportunities to stay current on best practices within the field of nurse case management.
  • Assist patients in setting achievable goals and monitoring progress to enhance motivation for adherence to care plans and return to work goals.
  • Maintain detailed records of patient progress, documentation of services and case notes.

Registered Nurse, Clinic Manager

  • Managed day to day operations of a clinic for six critical care physicians whose specialties are critical care, nephrology and pulmonology.
  • Triaged phone calls from patients, assessed medical status, and gave medical advice, including paging the physician when necessary.
  • I directed the medical assistants with medical issues, and assisted the physicians with patients being seen in the clinic.
  • I called prescriptions to pharmacies, set up out-patient procedures, provided injections, and monitored the overall medical care and needs of our critical care patients.

  • I was responsible for a caseload of 20 to 30 injured workers involving disputes regarding medical issues.
  • This position required analysis of the specific issue in dispute, obtaining all necessary information from the insurance company, interviewing the worker, the medical provider, the attorneys, and claims adjusters as necessary, and writing an appealable administrative order that directed how the issue in dispute was to be resolved.
  • This position required good communication skills, in order to explain the laws and rules to lay people, and good analytical thinking in order to make major medical decisions for people such as the appropriateness of a proposed surgery, or payment of outstanding medical bills based on the facts of the case and how the laws applied to each circumstance.

RN charge nurse/ Resident Care Manager

  • I started as a charge nurse, responsible for giving patient's prescribed medications, providing dressing changes, daily assessments of patient status, managing CNA staff assigned to my patients, calling doctors as needed, transcribing orders, and providing specialty care such as injections, running IV's, assessing surgical sites, managing ostomies, PEG, and NG tubes, as well as providing full body assessments and charting findings and procedures performed.
  • I then became the nurse manager of the Enhanced care unit, a 15 bed specialty unit of psychiatric/medically disabled patients.
  • My duties included all of the above, as well as coordinating the initial and quarterly MDS assessments, and initiating and managing care plans.
  • I also met weekly with the psychiatrist and social workers for updating treatment and care plans.
