- Planned individually-designed treatment programs to restore, maintain and improve physical functioning, alleviate pain, and prevent disability.
- Administer evidence-based interventions to improve mobility, strength, balance, and functional independence in patients.
- Monitor patient progress, adjust treatment plans as needed, and provide education to patients and their families on home exercise programs, injury prevention strategies, patient safety upon discharging to the home environment, as well as support continuity of care following hospital discharge to appropriate location
