Charlene Divoky

Brownsville,VT

Summary

Responsive Health Advocate with demonstrated experience providing outreach and support services to community residents. Established trusting and compassionate relationships with patients and families while offering education, assessment and social support to manage health conditions. Worked closely with medical providers to support comprehensive and coordinated care while facilitating ongoing follow-up and goal setting with patients and families.

My experience has included a "before retirement" and "after "retirement" phase. After moving to Vermont, in 2001 I became engaged in jobs that were primarily part time for a semi-retired person who wanted to stay busy and involved with meaningful activities. I was led to AmeriCorps/EpilepsyFoundationofNewEngland, where I learned a multitude of specialized skills that are very meaningful in helping others. I now plan to continue working as a Health Navigator. I have completed and received the official Certification for a Health Care Worker issued by the CDC.

I am totally open to questions that you may have, and look forward to an opportunity to introduce myself.
