Lpn for Joslin Diabetes Center Upstate Hospital. Rooming patients for Providers in an outpatient office setting. Responsibilities include: taking patients vital signs, going over their medications via electronic medical records, performing glucose and A1C testing, reporting any concerns from the patient to Providers, and downloading patients glucose monitoring equipment to place in their chart.

Worked on the Orthopedic Medical/Surgical floor. Passing medications, taking vital signs. documenting any changes with patients condition post surgery, wound care, personal hygiene assistance, monitoring fluid input and output. Assisted patients at all meals if needed. Reporting any concerns of patients to nursing supervisor if necessary.

Group Home setting providing all personal care to wheelchair bound individuals including: bathing, toileting or changing needs, cooking meals and preparing foods to proper consistency according to their dietary plans, passing medications and documenting on their MR sheets, documenting vital signs and any observed issues or changes into their chart, transporting individuals to and from all medical appointments and also to and from entertainment interests/events, advocacy for each individual unable to communicate their concerns when needed, and giving daily report to oncoming shift.
