Direct facility's assisted living program including recruitment of resident assistants, scheduling and training. In addition to staff training, I do assessments on potential residents to determine appropriateness for assisted living as well as determine the appropriate level of care they would require upon moving into the facility. I also maintain the face sheets and the charts with the most current information on assisted living residents. I do all assessments and care plans and update them quarterly. I am the only nurse in the facility on a daily basis and I'm on call 7 days a week 24 hours a day to be available to the residents, families and the resident assistants as needed.
I worked with 2 different med management companies doing third party med management in assisted living facilities around the state. Neither of those companies are in business anymore. I played a vital role in hiring and training nurses in several facilities to do med management and learn the computer program to document the meds given. I work closely with the third party med management companies in the position I am in at The Bloom.
I started out at National Homecare as a field nurse doing in-home assessments, lab, wound care and teaching. After 2 years with the company, they offered me the position as the Performance Improvement Coordinator (QA) because of my charting skills and eye for detail. I audited patient charts for visit frequency accuracy and read nurses notes to ensure that they were accurate with the plan of care specific to the patient and ensure that Medicare guidelines were being followed. I stayed in that position for 2 years then was offered a position the therapy department as the therapy coordinator. I received and distributed therapy referrals to the different disciplines and ensured that they were seeing patients according to the frequency on the plan of care and ensured that Medicare guidelines were being followed.
