COMMUNITY CONNECTOR (REMOTE)
Assist the County of San Diego in the emergency response to the COVID-19 public health crisis by providing over the phone, remote service to community members contacting 211 for information on COVID-19 testing, vaccines and general information. Assist clients with health insurance and affordable medical care options based on their unique circumstance and the eligibility requirements for each resource. Assist clients in locating emergency food assistance programs and nutrition resources. Assist clients with utility and rental assistance programs that are funded federally, locally, by non-profit organizations. Responsible for entering thorough documentation in Salesforce for every call. Connecting with other team members, supervisors, managers and directors as appropriate to best meet the needs of the clients.
