  • As a Program Coordinator, I collaborate intentionally with Youth Care Specialists, the Clinical Team, and community partners to design and implement targeted strategies that foster connections to youth-specific interests and joys. This approach ensures a holistic and supportive framework for their development, well-being, and personal growth. I engage directly with youth to understand their individual goals and interests, using this insight to identify and organize events and activities that align with their passions and aspirations. Through partnerships with local organizations, I ensure access to valuable resources and opportunities, fostering an environment that encourages personal growth, empowerment, and community engagement. My role emphasizes building trust and providing a safe, supportive space for youth to thrive, while fostering positive relationships with community stakeholders to create a network of care and opportunity.
  • Collaborate closely with the Connect LA Teams in Los Angeles, at the Youth Center, and Los Angeles County Service Planning Area (SPA) 4 to assess the needs of youth enrolled in the program, and identify appropriate interventions for their successful transition and stability.
  • Proactively inform the SPA 4 Youth CES Matcher of any program openings, ensuring timely and consistent communication to facilitate appropriate enrollments. Collaborate with CES housing teams to streamline enrollment and support for future participants.
  • Engage in internal and SPA-based CES Case Conferencing meetings to identify eligible participants, share information, and coordinate the most effective services for youth in need.
  • Regularly record and update participant goals, action steps, and progress towards their CLA goals in HMIS, ensuring accurate documentation and tracking of each youth’s journey.

  • ·As a Housing Coordinator, I assist Case Management teams in finding suitable housing for participants who are ready to lease. This includes identifying permanent housing options such as Section 8 and affordable market-rate housing throughout LA County.
  • Actively engage with property management companies, building relationships and staying informed about the latest housing market trends and rates to ensure housing programs can offer effective solutions.
  • Strong understanding of the surrounding cities and the cost of living, which allows me to match housing options with participants’ income levels, access to community support, and other essential factors like transportation. I also provide support to landlords by connecting them with case managers to ensure a smooth transition for program participants.

  • Provide support to Case Management team and as primary point support staff for youth as they prepare for a successful experience in attaining and/or maintaining tenancy in permanent housing through planning and crisis intervention. Develop a partnership with youth to identify appropriate permanent housing options such as subsidized housing Section 8, Shelter Plus Care, VASH, permanent supportive housing, affordable and market rate housing, and family reunification. Services also include providing linkage to resources such as Social Security, Medi-Cal, DPSS, and community resources.
  • Conduct Housing Navigation activities for members accessing housing through the Coordinated Entry System (CES). Document service reports using HMIS. Housing Navigation provides follow-up and retention services and provides back-up documentation in client’s file.
  • Serve as part of a team in delivering services to clients using the housing first model, trauma informed, and harm reduction model.

  • Provide individual service coordination and supportive services to assist members/residents in short term and long-term care plans in a diverse setting.
