Housing Case Manager works directly with clients, assisting them in identifying their housing needs; refers them to appropriate resources to assist with those needs, determining eligibility and financial support according to grant regulations and policies and facilitating their utilization of those resources
The Case Management goal is to empower clients and to promote individual, family, medical, physical, and emotional well-being
The Housing Case Manager first and foremost works directly with clients and their families and significant others
The Housing Case Manager is expected to interact on an as-needed basis with other staff, both paid and unpaid, of AIDS Response Efforts, Inc
When there are multiple Case Managers, close working relationships among all is expected
Direct supervision will be by the housing Program Manager and in their absence, by the Executive Director
The Housing Case Manager will also work routinely with other professionals in the community who represent medical, educational, legal, and social services agencies in the pursuit of appropriate services for mutual clients; commitment to serving vulnerable diverse populations and sensitivity to issues of confidentiality of patientcclient information
Evaluation of communication skills and cooperative attitude will be based on supervisory observation and feed-back, both solicited and unsolicited, from co-workers and other involved community service providers
Responsibilities may include, but not limited to: CLIENT SERVICE REPORTING - Completion of appropriate intake forms on all clients, and determination of eligibility for respective grant programs
Completion of Needs Assessments with call clients for the purpose of identifying areas in which the Case Manager can assist the client
Completion of Service Plans with for the purpose of outlining appropriate goals for the client - Case Management partnership to achieve, the steps required to meet these goals, and the time-frames of these objectives
COORDINATION WITH COMMUNITY HEALTH AND SOCIAL SERVICES AGENCIES - Coordination with community health and social services agencies for the purpose of assuring proper referral and follow-up of clients in need of such services

Keep HACCP documents and refrigerated load documents (receiving worksheet, bill of lading, and TTR report) separate for verifcation
Communicate temp abuse loads to the supervisor
Coordinate the move of the load to a dock door for inspection, and provide the temp abuse form to the unloadercreceiver
Download any TTRs (if included) on temp abuse loads
Forward the temp abuse form, photos of the load, and applicable TTR report to the RDC Claims Coordinator for product disposition
Coordinate geting the pallet trailers moved to a dock door for unloading upon request by Total Pallet Management (TPM) by selecting a particular trailer and changing its appointment time so a yard jockey receives a task through the Warehouse Management (WM) system to bring the trailer to the dock door
Receive the load of pallets into WM
After the pallet trailer has been unloaded or reloaded, close the load through WM, which will create a yard jockey task to move the trailer from the dock door
Communicate any trailer issues which need to be addressed to yard jockeys by relaying messages via hand-held radio
Print the 'Loads by Dock Doors Report' as a guide for trailersctemps
Responsible for compiling vendor compliance issues from warehouse receipts into WM for tracking purposes
Responsible for ensuring damages are noted on OS&Ds and have appropriate photographs of damaged product
