I received a list of ED patient discharges and would follow up with patients as needed regarding early signs and symptoms to report which would eliminate unnecessary ED visits. Educated patients on disease process and management. Assisted patients with accessing equipment, medications, physician appointments, home health, and mental health needs across the continuum of care.

The program was successful in decreasing unnecessary ED visits and provided quality care for patients to access necessary care as needed.
