CHESTINE DIZON

Renders nursing care to approximately 4 – 5 patients performing the following tasks / skills:

• Monitoring and assessing of patients conditions through regular patient care rounds
• Carrying out admission, transfer, and discharge
• Handling patients pre and post procedures and / or operations
• Providing care for patients on tracheostomy tubes
• Participating in multidisciplinary care rounds
• Preparing and administering medications and treatments and assessing for responses to care
• Providing health instructions
• Actively participates during hand off (e.g. shift to shift, transfer)
• Coordinates care through various support systems (e.g. radiology)
• Observes state and federal regulations during care of patients (e.g. HIPPA)
• Participates in staff meetings and performance improvement activities
• Acts as charge nurse
• Had been a nurse for 17 years with experience on general medical – surgical, psychiatric, neurological and rehabilitation patients. Recently assigned in a 20 bed capacity neuro-rehab unit.

• Neuro – Psychiatric Ward / Stroke Unit
