● Responsible for performing quantitative and qualitative reviews of the health record documentation
● Responsible for reviewing health records to ensure that all records contain sufficient information to meet the established documentation requirements as outlined by JCAHO, HCFA, VHA guidelines, FOIA, Privacy Act, Medicare Conditions of Participation and Medical Staff By-laws, as well as medical legal requirements
● Identify health record deficiencies and track electronic signatures. Effectively communicate, by person, internal email or telephone, any deficiencies to the appropriate clinical staff for correction
● Develop and compile written reports including provider and department specific information and hospital compliance reports regarding specific medical record reports
● Prepare complex weekly and monthly reports noting the statistical reports on data validation findings and the status of all incomplete/delinquent medical records and submit the reports to the HIM management and medical center leadership
Responsible for identifying problems concerning health information management, informing supervisor/management, and making appropriate recommendations for changes or solutions. Facilitate appropriate corrective action when authorized
● Ability to apply knowledge of medical terminology, human anatomy/pathophysiology, and disease processes to fully understand the content of the patient health record
● Ability to judge whether health records contain sufficient information for regulatory requirements; are acceptable as legal documents, are adequate for continuity of patient care
● Usage or organizational and time management skills in order to set work priorities and accomplish work within specified time periods
● Proficient in health information software, such as the electronic patient record, record tracking systems, chart deficiency systems, coding and abstracting systems, release of information systems (DSS software), document scanning technology, electronic spreadsheets, transcription systems, encoder products, electronic encounter forms and database software.
● Cross trained to backup Release of Information and File Room/Scanning departments
● Training and usage of Dragon Speaking software. Scheduling training sessions for software usage
● Provide efficiency and productivity of PALMS and to facilitate smooth and accurate flow of information, both clinical and administrative, within and outside of the VA Medical Center
● Transcribe all anatomic pathology reports, codes, edited and released to the provider with notification within one working day of receiving dictated/written pathologist's report. These anatomic pathology reports include surgical, cytologies, bone marrow, and autopsies which require knowledge of different procedures, formats, and medical terminology
● Complete appropriate sections of the Pathology send-out log and transcription or referred test results as they are received. In addition, routinely review log to identify potential missing reports. Follow up with the reference laboratory and report findings to the Pathologist
● Assist with patient registration in the outpatient area during times of high patient volume
● Validate laboratory fee-based charges. Resolve any discrepancies and issue payments within 30 days of receipt
● Process incoming/outgoing materials such as correspondence, reports and memoranda
● Compose letters and reports, review correspondence for accuracy and completeness, prepare public presentation outlines, and develop standard or form letters and replies to inquiries
● Schedule appointments, meetings, and conferences for Laboratory personnel
● Obtain and monitor the use of services, supplies or equipment for the office. Answer questions concerning policies and procedures related to support/office services and requests
● Assure laboratory obligations and fund control point budgets are maintained, current and available for supervisor and reports
● Provide training to new clerical staff on established procedures and practices particular to their individual duties and instruct on revised or new procedures as necessary
