Floor based-pharmacist for Pavillion Intensive Care, Oncology and 2West (med/surge area) (up to 60 patients)
Provide pharmaceutical care services to optimize patient therapy outcomes and improve medication regimens and patient safety through evidence based practices
Conducts comprehensive reviews of patient charts and incorporates pharmacy driven protocols including: renal dosing, IV to PO, therapeutic drug monitoring, antibiotic pharmacokinetic monitoring and adjustment, warfarin dose adjustment, and basal bolus correction
Active participant in ICU multidisciplinary rounds to discuss patient care plans
Conduct medication reconciliation upon admission to prevent medication errors
Collaborate with the healthcare team to provide recommendations for pain management
Contribute to antimicrobial stewardship efforts by monitoring antibiotic usage and making recommendations
Coordinate inpatient treatment regimens for chemotherapy with local oncologists and nursing staff
Review medication orders for accuracy and appropriateness based on patient-specific factors
Identify, document, and report adverse drug reactions and medication errors
Contributing member to Cancer Committee
Facilitates resident development through direct instruction, modeling, coaching, and timely formative feedback
Primary preceptor for PGY-1 residents on Internal Medicine Rotation and elective Critical Care Rotation
