Medical management of complex patients with multiple chronic conditions such as CHF, COPD, CKD, DM with goals of reducing hospitalizations and improving patients' quality of life. The Neighborhood Program was very successful at this goal.

Care of adult clients of all ages with a special emphasis of the geriatric client to medically manage diagnosis such as anxiety, depression, bipolar disorders and schizophrenia/schizoaffective disorder.
