• Provide end of life nursing care for in-home, adult family home and long-term care settings of patients residing in Dallas County, TX.
• Collaborate with Hospice Physician and ICT
• Symptom and medication management as per standing order of hospice physician
• Patient and family education and support of the end of life and bereavement process
• Provide end of life nursing care for in home, adult family home and inpatient settings of patients residing in Dallas, Collin, Denton, and Rockwall counties in Texas.
• Collaborate with Hospice Physician and ICT
• Symptom and medication management as per standing order of hospice physician
• Patient and family education and support of the end of life and bereavement process
• Coverage of after hours on call team responding to urgent need calls and death pronouncements.
• Provide end of life nursing care for in-home, long-term care and inpatient settings of patients residing in the East Dallas, TX territories
• Collaborate with Hospice Physician and ICT
• Symptom and medication management as per standing order of hospice physician
• Patient and family education and support of the end of life and bereavement process
• Monitoring of nursing students during their community health clinical rotations
• Oversee nursing operations and implement client care under supervision of the Psychiatric Medical Director
• Management and training of team of unlicensed personnel to maintain compliance and standard of care under the TX Administrative Code Title 25-Chapter 448
• Assess medical appropriateness, clearance, and mental status of each patient at time of admission, during residential treatment and at time of discharge.
• Provide appropriate referrals to higher or lower levels of care when indicated
• Reconcile all patient medications at time of admission, during residential treatment and at time of discharge.
