  • Triage Case Managers within the Triage navigator program at Hope Cooperative, also know as TLCS, worked as a part of a multi-disciplinary team to provide and coordinate services and resources for clients. The goal of the case manager at TLCS was to provide referrals, crisis intervention, support and encouragement to enable the client to access and use mental health and other services that prevent hospitalization and incarceration, and permanent housing and mental stability. I specifically worked at Loaves and Fishes location in downtown Sacramento at the only walk-in point to access services for the program. Loaves and Fishes is a Sacramento Hub for homeless services. At that location, it was just myself and a partner conducting crisis intervention and triage services for mental health and substance abuse. The overall goal was to provide rapid services that provided a wholistic approach to meeting their goals and needs.
