Reason for Leaving:
My reason for leaving AWFBMC was due to my indiscretion of medication diversion, self report to NCBON and voluntary enrollment in the Alternative Program for Nurses in Recovery. I have been in the program for more than 2 years with clean drug screening and reinstatement of RN licensure. FNP licensure is pending Joint Commission review in November 2025. To the best of my knowledge, I am not eligible for rehire.

My reason for leaving Gyn/Oncology was to return to reduce the number of days worked; from 5 days/week to 3 days/week. Completing the FNP
program required clinical practicum that couldn't be completed working 5 days a week. The same employer as above.
