I am the care coordinator for 60 - 100 members for CCA One Care and SCO members. I work with the skilled Nursing facilities on DC planning ensuring a safe discharge back to their community. i make sure they have all

Services in place. In addition I do all concurrent reviews on members who are skilled level

of care at the snf. I authorize their levels of care.
