Assists the Director of Wellness in the daily operations, clinical services, and administrative supervision of LPNs and HHAs of assisted living and memory care. Assists in recruiting, hiring, and ongoing training. Assists the Director of Case Management in admission, care coordination, and discharge planning. Assists in developing Individual Service Plans (ISP) and investigates grievances and incident reports.
Responsible for the facility’s infection prevention and control program (IPCP) to help prevent the development and transmission of communicable diseases and infections based on the Joint Commission, CMS, OSHA and state regulations. Developed and maintained the IPCP policies and procedures and conducted assessments, education, and guidance to staff, residents, and family members. Participated in quality assurance (QA) initiatives and led the vaccination programs.
Unit Manager - Assists in evaluating the patient’s condition and administering ordered treatments in LTC and sub-acute. Assists the unit with daily operations, clinical services, and overseeing the unit’s RNs, LPNs, and CNAs. Responsible for maintaining care standards, perform diagnostic tests, and monitoring patient and recovery care. Communicates with doctors, patients, and family members. Assists in admission, care coordination, and discharge planning.

Assists in recruiting, hiring, and ongoing training. Triage patients especially on a walk-in basis. Vaccinations and teaching visits. Develop and implement staff schedules. Assists in prior authorizations and care coordination.
LPN - Staff - Responsible for screening and performing procedures within scope of practice. Screening involves vital signs, interviewing, counseling, discharging, and teaching patients along with proper documentation of clinical observations and findings. Procedures include (but not limited to) EKG’s, hearing and vision tests, blood glucose checks, immunizations, inhalation treatments, blood draw. Assists providers with examinations.
Assessment, data collection, participated in planning and executing the plan of care. Responsible medication administration, tracheostomy suctioning and tracheostomy care for both ventilator and blowby patients, urinary catheters, GT site care, wound care, colostomy care, ostomy care, bedside care along with CNAs.
