Working with Hospital Referral portals across the west region , Texas, Arizona, California, Oregon, Washington, Colorado, Nevada. Responding timely to referral request, Accepting or place in pendng status, communicating with hosp Case managers if needed, Collecting patient information emailing info to Intake and sales rep at appropriate Centerwell Branches.

2020-2022 Every 3 months a Follow up phone call placed to previous patients checking needs for possible home health needs for Nursing or PT/OT : skin care/wounds, home safety/falls , med teaching, Contacting PCP for orders to eval for home health.
