Assist Polk County Public Health by providing public health nursing services utilizing standing orders and protocols in multiple programs:

Clinical services;

· Reproductive health/STI by utilize standing orders and

protocols to provide STI screening, treatment and education

for clients; utilize standing orders and protocols to provide

birth control options for clients, client education and

referrals

· Immunizations: utilize standing orders to provide

immunizations to clients, client education and referrals

Communicable disease:

· provide assistance in identifying and investigating

reportable communicable diseases and tuberculosis cases

· conduct telephone case interviews, contact tracing

· act as a consultant in education and prevention strategies.

Breastfeeding Coordinator:

· provides leadership to the local WIC agency regarding

breastfeeding promotion, education and support.

· provides basic breastfeeding information and

encouragement to WIC pregnant and breastfeeding

mothers

· supporting parents and infants on their breastfeeding

journey

· act as Breast Pump Coordinator

Supervises the nursing programs and staff within Public Health.

Performs administrative, management, and training duties.

Works with the community to assess and coordinate programs.

Serves as part of a multi-disciplinary supervisory team.

Performs clinical duties of a Registered Nurse as needed.

In compliance with CDC and Oregon Health Authority, develop and implement COVID protocols to ensure community safety and wellbeing throughout the pandemic.

Member of multidisciplinary and community partner committees working toward ensuring equitable COVID vaccine distribution throughout Polk County

• Work with Nursing Supervisor on policies/procedures to assure current compliance requirements are met
• Based on program requirements, maintain records, perform required data collection for projects and produce required reports
• Provide expenditure information to supervisor to be used in budget, grant preparation, and expense monitoring
• Adhere to standards and directs nursing practices to ensure high quality professional service to the community and their health needs
• Plans and exchanges information with other community health providers relating to case management of clients involved in HIV case management
• Represent Polk County Public Health through various community outreach events
• Examples include vaccination clinics throughout the community, school information events, and Homeless Connect outreach events
• Participate in building relationships with other community health partners to improve health outcomes for Polk County residents
• Collaborate with community health providers, partners and agencies to enhance health outcomes and promote community health awareness
• Provide nursing services in various Public Health programs including Vaccine Program Coordinator, HIV RN Case Management and Reproductive Health
• Provide program training to new nursing staff
• Provide back up for clinical nursing staff as needed
• Performs related duties as required.

• Prepare adult and child clients for sessions with medication Prescribers
• Complete weight, blood pressure, review lab results and other required checks prior to each medical appointment
• Educate clients and/or family, and consult with pharmacies, regarding obtaining medication usage, dosaging, and possible side effects
• Work with clients to understand mental health and physical health relationship
• Assist clients in obtaining medication through special services
• Advise and assist clients in obtaining discounted community services and programs
• Provide complete and accurate charting of patient information and relaying patient concerns and care needs to nursing staff.
