First thing first, I've been working the social service field for over eight years. I have adequate experience in working with children and adults of all ages, ethnicity, cultures, socioeconomic statuses, and medical conditions. Also, I posses the ability to lead a team and convey ethical and moral values to the organization and its consumers. Lastly, I am highly ambitious to succeed and climb the latter to prosperity.

Prepares and presents relevant information for annual IP's for assigned residents
Develops, monitors, and implements skill developments for individual plans (IP's) of
assigned residents in accordance with interdisciplinary team input, Medicaid and
Licensure regulations and facility-prescribed format
Prepares and presents quarterly reviews
Prepares and maintains monthly progress notes on assigned residents
Initiates Comprehensive Functional Assessment (CFA)
Collects and prepares data
Coordinates and participates in interdisciplinary team meetings
Works directly with assigned residents on skill developments; must maintain flexible
scheduling to meet resident needs
Coordinates and participates in resident community outings and leisure activities
Shops for resident related items based on individual resident need
Provides transportation for community outings, appointments, outside programming etc.
Initiates and facilitates team process
In-services programming staff in proper programming methods and keeps them informed
of resident skill developments
Coordinates Heinzerling programs with out-of-building programs to ensure consistency
Selects and adapts appropriate materials as related to skill developments
Participates in initial, required and ongoing staff development trainings and in-services
Monitor clients overnight at a developmental disability home.
Dispense medication to clients.
Cook meals and clean for the clients.
Record daily progress and reports of the clients.
Transport clients to necessary appointments, school, and programs.
