A team player, with compassion and a desire to help others and over 3 years of Program Management person-centred planning and provision of services experience responsible for the development, coordination, implementation, and monitoring of services provided in the ("Home and Community-Based Services" Medicaid waiver program serving adults with developmental and intellectual disabilities) and related conditions residing in their own homes with family members, a host provider or in company owned supported living homes
As a program Manager I am charged with overseeing all overall operations and services provided in 6 small HCS group homes serving 30 plus individuals with intellectual disabilities. General Responsibilities include:
· Regulatory compliance with HCS certification principles and requirements
· Management and Supervision of Direct Care and Professional Staff in coordination with Program Director
· Monitoring of specialized documentation generated by all program staff
· Develop and maintain budgets, timelines, and performance metrics as set by company Executive Administration
· Oversight of physical plant and maintenance • Liaison to regulatory authorities
· Ongoing communication with Program Director
· On-call duties and flexible schedule to address emergencies
· Conduct regular staff meetings to ensure program objectives are met.
· Provide guidance and support to direct support staff in the implementation of program goals.
Primary Responsibilities include:
· Directs personal assistance with daily living activities such as grooming, eating, bathing, dressing, and personal hygiene
· Participates and assists in the development of the Personal Development Plan (PDP) for the individuals served
· Ensures PDP is implemented as it relates to residential services
· Reports to the Interdisciplinary Team (IDT) any problems, proposed program changes, behavior of individuals served, concerns, training needs, medical concerns, dietary issues, etc.
· Ensures documentation of behaviors, incidents and injuries, program data collection, medication data, and progress notes are completed accurately and in a timely manner
· Assists with activities of daily living, meal planning and preparation and housekeeping
· Assists with ambulation and mobility as needed
· Secures and provides transportation
· Reinforces counseling and therapy activities
· Assists with self-administration of medications and the performance of RN delegated tasks
· Supervises and assures individuals' health, safety, and security
· Facilitates inclusion in community activities, use of natural supports, social interaction, participation in leisure activities, and development of socially valued behavior
· Reports incidents/injuries in a timely manner
· Cooperates in internal/external investigations

Adept with email and word processing programs

Excellent Program and time management skills,
