  • Infectious Disease Elective - Collaborated with infectious disease physicians and recommended appropriate treatments based on culture & sensitivity and individual patient factors. Updated hospital antimicrobial stewardship policies and order sets to reflect the most current guidance.
  • Acute Care - Adjusted dosing of vancomycin and gentamicin per pharmacokinetic protocol and processed other inpatient medication orders, counseling providers when appropriate. Conducted VTE screening, medication reconciliations, and immunization screenings for newly admitted patients. Compounded sterile IV medications for inpatient use.
  • HIV/Hepatitis C - Ordered regularly scheduled labwork, immunizations, and refills, recommended medication interventions, and provided counseling. Contacted patients based on high-risk criteria as part of an outreach campaign to educate and spread awareness of PrEP services.
  • Leadership & Management - Shadowed the Chief Pharmacist and outpatient pharmacy manager during their administrative duties, such as ordering inventory, holding job interviews, and meeting with hospital leadership. Analyzed the most often requested, approved, and denied non-formulary medications to create a report used to determine the estimated annual budget.

  • Rheumatology/Neurology - Collaborated with specialists to devise appropriate treatment plans and sign patients up for patient-assistance programs to obtain cost-prohibitive biologic medications when needed. Created educational materials on available biologics and their indications for internal use. Provided medication counseling and self-injection training to patients in visits, as well as regular telephone monitoring.
  • Specialty Pharmacy & Tobacco Cessation - Screened, verified, and dispensed prescriptions for the dermatology, wound care, ophthalmology, and Ear/Nose/Throat clinics, reaching out to specialist providers for medication interventions or prescription issues. Counseled patients and prescribed appropriate nicotine replacement products or Chantix under a collaborative practice agreement in the pharmacist-lead tobacco cessation clinic.
