Provides and facilitates the activities of daily living for patients in accordance with the plan care and dialysis unit flow. Addresses any patient issue with the nurse. Responsibilities include transporting patient to and from treatment area; collecting stool, urine or sputum specimens; assisting with patients nutritional needs, takes down used dialysis disposable equipment; cleaning and/or disinfecting dialysis machines and station of hemodialysis machine; taking vital signs; EKG’s, obtaining patient weight and records. Assembling equipment for nursing staff to initiate treatment. Reports machine malfunctions to service company immediately. Assists with discharging of patient as instructed and guided by RN. Performs and records daily monitoring of water treatment system, operating perimeters, to include total chlorine testing.
Assists with patient care treatments utilizing aseptic technique and universal precautions as per WUH policy and procedure. Assists with cannula needle removal and aseptic bandaging of needle sites.
Responds to customer needs. Ensures completion of patient care assignment in an organized, efficient manner. Assists patient, family, visitors, and health care team members by answering questions, delivering messages and giving information within the scope of position. Reports all changes in patient condition and pertinent patient information to the nurse, manager or designee promptly.
Recognizes and responds to issues that may cause problems and/or refers issues through proper chain of command utilizing the escalation policy. Recognizes the family as defined by the patient and incorporates family centered care in daily practice. Documents legibly. Maintains open channels of communication through intra and inter shift reporting.
Promotes and maintains patient safety. Maintains call bell within patient reach. Answers patient calls promptly. Orients and re-orients patients to the patient care unit as needed. Responds to emergency situations as appropriate. Utilizes proper body mechanics when moving patients or equipment.
