Manage heart failure program for the Central Region Cardiology Department. Triage phone calls and relay symptoms and change in conditions to providers. Make follow up telephone calls to manage patient conditions at home. Room patients in clinic, take vitals, teach heart failure guidelines. Prepare medication requests for refills. Train and over see clinical staff in our department.
