  • Collaborated with other Multidisciplinary Department Managers about planned Patients/Residents Admissions from the hospital or community. The likelihood of this person needing rehabilitation services or longer skilled services, relies solely on that persons hospital physician as well as their responsible party. This extended care will assist in allowing that person to have a better quality of life and live as independently as possible.
  • Attended daily management meetings, conferences and marketing venues in relation to new strategies for new admissions , as well as quarterly townhall meeting. This townhall venue was implemented to keep our families as well as our seniors in the community informed on any or new changes in Long Term Care, Medicare, Hospice and much more.
  • Implemented a user friendly tool for documentation, this would allow the Social Worker to differentiate each residents short term and long term needs. This tool would clearly define a persons cognitive skills for daily decision making ability, mood and behavior pattern prior to admission or their extended stay at the facility. This system is in conjunction with what sets the treatment plan and length of stay. As a consultant is was a responsibility to assess what needed to be addressed from 2016 & 2017 annual survey that targeted the residents rights, dignity and behaviors. The facility was putting together their action plan for correcting the last state survey of concerns.
