Work with a multidisciplinary team as a float case manager to complete psychosocial assessments and develop safe discharge plans for patients and families on all hospital units as needed.

Provide after hours case management support to all areas of the hospital as needed.

Assisted with the development and implementation of a new hospital program. Worked as part of a small team to assure compliance with Medicare guidelines relating to admission status and criteria for admission.

Led the development and implementation of a health improvement program focused on the diabetic population.

Participated in a statewide collaborative involving partnering with physicians and their patients to develop office practices aimed at improving their diabetic patient's overall health outcomes.

Assisted clerical staff with the creation of aggregate reports as they related to these patients.
