• Led comprehensive discharge planning for Veterans in acute medical/surgical inpatient units, collaborating with interdisciplinary teams (physicians, nurses, and other providers) to assess psychosocial needs, develop individualized transition plans, and ensure safe, timely discharges to home or appropriate community settings.
• Coordinated post-discharge care linkages with community providers, including Assisted Living Facilities (ALFs), Skilled Nursing Facilities (SNFs), shelters, and other resources, to address housing, support, and long-term care barriers and promote seamless continuity of care.
• Facilitated completion of advance directives and related discussions with Veterans and families, advocating for their preferences in end-of-life and medical decision-making while integrating these into discharge and treatment plans.
• Conducted psychosocial assessments and crisis interventions as needed, working closely with the treatment team to resolve barriers, secure necessary supports (e.g., transportation, home health), and minimize readmission risks for high-acuity Veterans.

• As a BSW Intern was offered Job handling military veterans within the criminal justice system and are in need of a public defender. Work with attorneys and social services department to find benefits available to veteran clients and programs to aid in legal mitigation process. Responsible for contacting clients both in and out of correctional facilities in Circuit 9 courts for the state of Florida.
