  • As a TOC nurse I am responsible for following our dual eligible members who go into the hospital for an acute or chronic condition until there transition to home. I make an outreach once in the hospital to the member and CM at the hospital. Once discharged, I call the member within 48 business hours to reach for the Post discharge assessment and if needed, complete the Annual health risk assessment. I review and update the care plan, update medications, ensure member understands there discharge paperwork, has follow up appointments set up and transportation. I assist members, as needed with these calls to ensure they have follow up appointments scheduled along with transportation. I make a follow up call 14 days after discharge to check back in with our member's, ensure they went to appointments and assist with any needs they may have. I check report daily for new inpatients that we will need to follow. In January of 2022 I had one my member's story published in the CESI Member Experience Story webpage. I was recently voted Coach of the year in October 2024.
