During my position as staff, charge and supervisor, I was the referral person for patients needing to go to bone marrow transplant. Which required reviewing of charts to obtain necessary records of labs, biopsies, treatment regimens, provider notes, etc, to send to the transplant facility. Then I would follow up on patient that returned post-transplant for when vaccines were needed and scheduled accordingly. I developed my own templates/flowsheet for post-transplant vaccine and follow up to keep track of patients for compliance.
