Provide patient care and education to patients with chronic illnesses.
Stock clinical workstations and medication carts with necessary supplies.
Contact physicians with change in conditions, accept orders, enter orders into the Point Click Care system.
Accept new admissions and conduct assessments utilizing appropriate documents/forms.
Collaborate with social worker, dietary, mental health and other facility team members regarding resident status.
Follow facility and state policy and procedures. Follow safety procedures.
Practice open communication with unit and facility management.
