Facilitates evaluation of records by physicians, peers, and quality standards, according to protocols and receives and implements constructive directives.
• Performs Acute Care and referral to in-patient status from Cardiology Clinic and telehealth-remote care if necessary.
• Reviews Fax, Email, and E-Consults as needed.
• Conduct initial triage on all consultations/referrals and prioritize accordingly.
• Call patient to review all available care options including clinic versus acute care.
• Document the conversation with the patient using consult messaging.
• Provide each patient with information about all appropriate care options available, including in-person, virtual and telephone.
• Determine eligibility and secure patient appointments when appropriate.
• Coordinate with clinical and administrative staff who have training in both in-patient and out-patient scheduling processes to eliminate unnecessary steps and persons from the process – making it easier and quicker to schedule.
