Direct nursing care in primary care setting to clinic patients via telemedicine, virtual and face to face care utilizing triage, procedures, nurse managed protocols and medication management. Collaborate with case management to manage care, treatment planning, transfers, and discharge plans. Contribute to the redesign of report sheets utilized for patient handoff and re-scripting discharge instructions to minimize the risk of re-hospitalization or adverse outcomes. Educate patients on diagnosis to minimize repeat admissions. Participate and administer regular in-service programs for the continuing education of staff.

Admissions nurse, the first point of contact for patients moving to hospice care. Guide patients and their families through initial assessments, the admission process, education regarding expectations of the process of dying and their individual situation. Provide information, pre-assessment, treatment planning, wound care, and admission services to patients. Conduct pre-assessment visits in the home or health care facility as necessary. Communicate with the Intake Department, the referring physician, and Hospice Medical Director to assure appropriateness of each referral and those essential certifications. Coordinates care of patients with various disciplines and community resources, both internal and external, in a fiscally responsible manner.

Assessed, planned, and implemented nursing care to meet the patient's needs for women and infants’ postnatal population. Provided expert-level, recovery-focused acute care for women and their newborns, ensuring comfort, safety, and treatment efficacy to foster a smoother transition for families.
