With over 30 years of experience as a registered nurse, I have worked in various medical settings. Throughout my career, I have honed my skills in visiting, assessing, and providing care to patients in their homes, as well as conducting telehealth visits by phone. My expertise lies in identifying patient needs and implementing tailored interventions to address those needs effectively. Recently, I have taken on a significant role in educating and coaching patients, families, and caregivers on disease processes and symptom management to improve their overall quality of life. Effectively communicating patients' medical conditions and symptoms to providers is a process that I am highly skilled in and comfortable with. Additionally, I am well-versed in the community resources available and excel at assisting patients in connecting with the appropriate ones. Collaborating with a team of medical professionals to develop comprehensive care plans for individual patients is an area where I have gained valuable experience. Currently, I am seeking a hybrid or remote position that will allow me to utilize my extensive experience and skills to make a positive impact on the quality of patients' lives.
• Nurse Case Manager for patient’s living with a serious or life limiting illness proving home and telephonic visits focusing on enhancing quality of life.
• Complete a comprehensive assessment of individual patients in including physical condition, health goals and needs and make recommendations to providers to manage distressing symptoms and improve quality of life.
• Work in collaboration with a team of professionals including Social Workers, Physicians, Nurse Practitioners, Chaplains, Volunteer Coordinators and Volunteers to develop a plan of care encompassing physical, social and spiritual needs.
• Provide education to patients, families and caregivers on disease management, symptom control, and coping mechanisms for illnesses such as Congestive Heart Failure, Diabetes, COPD, Chronic Kidney Disease, Parkinson’s Disease, Alzheimer’s Disease, Cancer and other chronic illnesses.
• Identify needs and assist patients to connect with community resources such as Home Health services, In-Home caregiving agencies, durable medical equipment, financial services,
Nutritional services, and more.
•. Provide assistance to clarify health goals and develop end of life plans, transitioning patients to hospice care when appropriate.
