-Serves as a Licensed Practical Nurse in Clinical nursing department.
-Monitors medical readiness of Active Duty Service members and families with health conditions affecting deployment readiness, including physical, development, emotional, and cognitive illnesses.
-Conduct administrative review of service members and families' medical records, unitizing Army healthcare systems to include JLV (joint legacy Viewer) and MHS Genesis.
-Identifies and understands the implications of acute and chronic
conditions affecting medical readiness status.
-Provide patient and family education on their medical condition, medical processes, an lifestyle changes to promote and improve health and wellness.
-Assist in coordination scheduling and follow up appointments for soldiers and families.
-Check in soldiers and family members, document in MHS-Genesis history of patients.
-Obtain vital signs an collaborate and communicate with unit healthcare providers and team RNs on soldiers and family healthcare needs.
-Coordinate and provide continued education for soldiers and family members for due and overdue PHAs (physical Health assessments) and HEDIS metrics.
-Ensure accurate submission fo time into ATTAPS and DMHRSI time keeping system.
-Excellent communication skills to effectively coordinate care between soldiers, family members, provides, supervisors and other team members.
- Participates with staff moral committee events.
-Shadow and assist 36 hours monthly as Forensic Healthcare Assistant.
