Management of patients requiring extended treatment for health needs related to medical/surgical, orthopedic, neurological, or burn issues. Provides direct patient care to caseload of 3-8 patients, from completion of admission documentation to discharge. Assist with daily personal care, management of IV therapy, administration of medications, and wound care as needed.
Establishes and documents a plan of care, including problem identification/interventions needed, patient education needs, and desired goals/outcomes for each patient.
Appropriately interprets and utilizes diagnostic, laboratory, and monitoring data in patient care.
Works closely with multidisciplinary team to help enhance the patients’ overall rehabilitation experience. Communicates objective and subjective data regarding patient/family with appropriate members of the multidisciplinary team.
Functions as a preceptor/mentor for new nursing personnel assigned to the unit.
Delegates, guides, and directs unlicensed health care providers and LPNs in task and is responsible for the care provided according to their documented competencies and/or scope of practice.
Facilitates communication of pertinent information with and/or makes rounds with physicians.
Completes/maintains all hospital and nursing competencies as required.
Managed hospice team inclusive of 15-17 RNs, LPNs, Aides, Social Workers, Clergy and Massage Therapists in providing in-home hospice care services
Managed RNs and Aides providing in-home skilled care services
Responsibilities encompassed oversight of all aspects of Baptist Homecare and Hospice-Tipton.
Managed sixteen staff members including assignments, staff development and training, team building, problem solving, conflict resolution, and other staff related issues encountered. Also responsible for direct reporting of agency activities to the Executive Director System Home Care and functioned as a resource person for the entire Baptist Home Care system
