• Aids with care in an LTC/ACUTE/TCU/POST OP, MC and HOSPICE settings providing professional care to patients assisting with personal care such as showers, dressing, peri-care, vitals, medication technician assistance (AFH) and (MC) units, along with documentation, meal preparations and assist with light housekeeping.
• Aids with terminal ill clients by monitoring patient’s conditions, maintaining comfort during the final stages of life and provide support to patients and families in a variety of care environments.
• Collaborate with by informing the care team (physicians, nurse, social workers, chaplains and others) planning, preparing and adjusting plan of care as needed.
