Case Management for foster case load up to 18 cases. Assigned to overflow Intensive Services Foster Care (ISFC) youth for Riverside and Palmdale Regions. Ongoing assessment of case load (youth) and ongoing assessment to ensure Resource Parents (RP’s) and youth are a good fit. Ongoing Case Management to ensure youths needs are met and ensure RP’s are in compliance with responsibilities to youth, their files and trainings are up to date. Provide Weekly visits to youth (Two weekly contacts for ISFC youth). Ongoing collaboration to ensure youth have weekly and or court ordered visitation with Biological Family. Carried out Quarterly Team Meetings for youths Initial Reports and Quarterly Reports to County Social Worker, Monthly Team Meetings for ISFC youth, obtain Monthly Reports for all youth, obtaining monthly height and weight for all youth, record youth’s health, development, living conditions, adjustment, socialization, school performance and relationship with biological family on a regular basis. Document and report issues with biological family regarding visitation/phone calls. Provide ongoing guidance and training to the Certified Resource Parents. Promote the County, CCL guidelines, Out of Home Care and McKiley policy and procedures. Communicate with respective County Social Worker and County Social Worker Supervisor, as needed to exchange information and provide updates on the youth and their well-being. Produce SIR as indicated and in a timely manner. Provide support to Certified Resource Parents 24/7 for managing all issues and crisis. Rotate the After Hours/Crisis Line (approx. every 2.5 months) and provide intervention warranted to stabilize the situation so youth and RP feel safe and agree to continue to collaborate and work together. Responsible for yearly training requirements to maintain the Social Worker position.
