Provided in-person outreach and intensive case management to homeless elders to assist them with obtaining safe, stable housing and appropriate support. Served a revolving caseload of older adults in shelters or unstable housing, working closely with them to work through barriers to housing and work on other goals determined by a person-centered care plan. Conducted training for internal and external providers on working with homeless older adults.

Supported individuals in mental health management by identifying and practicing coping skills and alternative methods. Taught and modeled community integration skills such as self-managing medications, managing personal finances, appropriate interpersonal interactions, managing the public transportation system, completing the necessary home maintenance tasks, and accessing community resources. Educated healthy living habits and developed the accompanying skills.

Supervised individuals to ensure their safety and well-being. Provided medications and other treatment to individuals under the instructions of the treatment plan. Assisted individuals with daily activities, including eating and movement. Intervened in crises and trained to restrain individuals who may become physically violent.

Supported individuals under 18 gain access to medical, social, educational, vocational, and other services connected to mental health needs. Collaboratively consisting of formal service providers and informal community supports closely connected, professionally and personally. Participated in a wraparound approach to intensive, individualized care coordination involving a team process that wraps services, supports, and resources around a child or youth with a severe emotional or behavioral disorder to meet goals set by the team.

• Delivered personalized educational, behavioral, and emotional support to individual students to enable positive learning outcomes. Offered student support through special accommodations, extra assistance, and assessments. Collaborated with teachers to create customized classroom environments integral to students' needs.

Coordinated scheduling ensures service delivery in a single-family home and assists with continuous quality improvements in service management and compliance with 245D licensing regulations. Supervised day-to-day operations to be carried out following the care plans and the intentions of those living in the home. Contributed to the successful admissions and program planning process at the direction of the Residential Program Administrator.
